Former patients may request copies of their medical records in writing to admin@cortexon.io, or by mail to Cortexon Medical Corporation, 3790 El Camino Real #1225, Palo Alto, CA 94306. Please include your full name, date of birth, and the address or email where records should be sent. Requests are fulfilled within 15 days, consistent with California Health & Safety Code §123110.

Dr. Jacobs continues in clinical and academic roles at the VA Palo Alto Health Care System and as a clinical assistant professor (affiliated) at Stanford University School of Medicine. He is not accepting private patients.
